Below is a list of religious exemption examples that have been used by others in the community. These have been submitted by actual followers of OFA! Of course, exemptions may vary from workplace to workplace so this is NOT a guarantee, but hopefully this will help get you off your feet. You should contact your local church to help you further.

To whom it may concern,
I want to start off by saying thank you for allowing me to submit my personal attestation. I understand that this is a difficult time for all. I am writing today from the bottom of my heart to request a religious exemption from the COVID vaccine, as receiving this vaccine is against my sincerely held religious beliefs.
It is my genuine and sincere Christian belief and faith that my body is a temple of the Holy Spirit and I have God given responsibilities and requirements for me to protect the physical integrity of my body from unclean foods and injections. It is my conviction to keep this temple clean by not introducing things the bible considers unclean. In recent understanding of the vaccine development, production and ingredients receiving the COVID-19 vaccine would be morally compromising.
1 Corinthians 3:16-17 “Don’t you know that you yourself are God’s temple and that God’s spirit dwells in you? If anyone destroys God’s temple, God will destroy that person; for God’s temple is sacred and you together are that temple”.
1 Corinthians 6:19 “Do you not know that your bodies are temples of the Holy Spirit, who is in you, whom you have received from God? You are not your own”.
Myself and my family have remained healthy throughout this entire pandemic. We continue to be aware of our surroundings, practice hand hygiene, wear masks and practice social distancing. My request is that I resume all my normal duties and responsibilities. By doing, so I will use all necessary precautions for my own safety and the safety of others.
